Technological advances have directly impacted the way science is communicated, reducing borders and connecting individuals, but the language barrier still persists as a significant obstacle to collaboration between researchers and the dissemination of knowledge. Publications in English have a wider reach, making it possible to disseminate the results of studies and research in national journals. ABSTRACT

ABSTRACT Purpose: This study aimed to analyze the association between magnetic resonance imaging apparent diffusion coefficient map value and histopathological differentiation in patients who underwent eye enucleation due to retinoblastomas. Methods: An observational chart review study of patients with retinoblastoma that had histopathology of the lesion and orbit magnetic resonance imaging with apparent diffusion coefficient analysis at Hospital de Clínicas de Porto Alegre between November 2013 and November 2016 was performed. The histopathology was reviewed after enucleation. To analyze the difference in apparent diffusion coefficient values between the two major histopathological prognostic groups, Student's t-test was used for the two groups. All statistical analyses were performed using SPSS version 19.0 for Microsoft Windows (SPSS, Inc., Chicago, IL, USA). Our institutional review board approved this retrospective study without obtaining informed consent. Results: Thirteen children were evaluated, and only eight underwent eye enucleation and were included in the analysis. The others were treated with photocoagulation, embolization, radiotherapy, and chemotherapy and were excluded due to the lack of histopathological results. When compared with histopathology, magnetic resonance imaging demonstrated 100% accuracy in retinoblastoma diagnosis. Optic nerve invasion detection on magnetic resonance imaging showed a 66.6% sensitivity and 80.0% specificity. Positive and negative predictive values were 66.6% and 80.0%, respectively, with an accuracy of 75%. In addition, the mean apparent diffusion coefficient of the eight eyes was 0.615 × 103 mm2/s. The mean apparent diffusion coefficient value of poorly or undifferentiated retinoblastoma and differentiated tumors were 0.520 × 103 mm2/s and 0.774 × 103 mm2/s, respectively. Conclusion: This study revealed that magnetic resonance imaging is useful in the diagnosis of retinoblastoma and detection of optic nerve infiltration, with a sensitivity of 66.6% and specificity of 80%. Our results also showed lower apparent diffusion coefficient values in poorly differentiated retinoblastomas with a mean of 0.520 × 103 mm2/s, whereas in well and moderately differentiated, the mean was 0.774 × 103 mm2/s.

ABSTRACT

ObjectiveTo investigate the preventive and therapeutic effects of Tiaogan Huaxian pills combined with entecavir on hepatic fibrosis in chronic hepatitis B with liver Qi stagnation, spleen deficiency, and blood stasis syndrome and its effect on diffusion-weighted imaging (DWI). MethodClinical data of 117 patients with liver disease who visited the Department of Hepatology at the First Affiliated Hospital of Guangxi University of Chinese Medicine from January 2021 to April 2022 were retrospectively analyzed. According to different treatment plans, they were divided into a control group (59 cases) and a treatment group (58 cases). Both groups of patients received entecavir-based etiology treatment, and the treatment group added Tiaogan Huaxian pills on the basis of basic treatment. Both groups were treated for 24 weeks. Before and after treatment, the two groups were compared in terms of alanine aminotransferase (ALT), advanced surgical technologies (AST), total bilirubin (TBil), hepatitis B virus (HBV)-DNA conversion rate, liver stiffness measurement (LSM), four items of liver fibrosis (hyaluronidase, type Ⅲ pro-collagen, type Ⅳ collagen, and laminin), the fibrosis index based on four factors (FIB-4), the aspartate aminotransferase to platelet ratio index (APRI), the apparent diffusion coefficient (ADC) value in magnetic resonance imaging (MRI), and traditional Chinese medicine symptom scores, so as to analyze the efficacy of the two groups. ResultBefore treatment, there was no significant difference in ALT, AST, TBil, LSM, four items of liver fibrosis, FIB-4, APRI, HBV-DNA conversion rate, ADC value, and traditional Chinese medicine symptom scores between the two groups. After treatment, both groups of patients showed significant reductions in ALT, AST, TBil, LSM, hyaluronidase, type Ⅲ pro-collagen, type Ⅳ collagen, laminin, FIB-4, and APRI (P<0.05) and a significant increase in ADC value (P<0.05) and HBV-DNA conversion rate (P<0.01). The traditional Chinese medicine symptom score of the treatment group decreased significantly (P<0.05). Compared with the control group after treatment, the effective rate of clinical traditional Chinese medicine in the treatment group was 91.38% (53/58), which was significantly higher than that of the control group (54.23%, 32/59) (Z=-4.325, P<0.01). In the treatment group, ALT, AST, TBil, LSM, hyaluronidase, type Ⅲ pro-collagen, type Ⅳ collagen, laminin, FIB-4, APRI, and traditional Chinese medicine symptom scores all decreased significantly (P<0.05), and the increase in ADC values was more significant (P<0.05), while the difference in HBV-DNA conversion rate was not statistically significant. There were no serious adverse reactions or events in either group. ConclusionTiaogan Huaxian pills combined with entecavir have significant clinical efficacy in the treatment of hepatic fibrosis in chronic hepatitis B, which can reduce liver inflammation activity, delay hepatic fibrosis progression, and reduce traditional Chinese medicine symptom scores. It is worthy of clinical promotion and application.

El campo de la radiología ha experimentado avances notables en las últimas décadas, con desarrollos que van desde la mejora de la calidad y digitalización de las imágenes hasta la detección asistida por computadora. Particularmente, la aparición de técnicas de Inteligencia Artificial basadas en Deep Learning y Visión Computacional han promovido soluciones innovadoras en el diagnóstico y el análisis radiológico. Se explora la relevancia de los desarrollos y modelos open source en el progreso de estas técnicas, resaltando el impacto que la colaboración y el acceso abierto han tenido en el avance científico del campo. La investigación tiene un enfoque cualitativo, con alcance descriptivo y retrospectivo, de corte longitudinal. Se realizó un análisis documental de la evolución y el impacto del open source en la Radiología, poniendo de relieve la colaboración multidisciplinar. Se examinaron casos de uso, ventajas, desafíos y consideraciones éticas en relación con la implementación de soluciones basadas en Inteligencia Artificial en Radiología. El enfoque open source ha mostrado ser una influencia positiva en la Radiología, con potencial para influir en la atención médica, ofreciendo soluciones más precisas y accesibles. No obstante, se presentan desafíos éticos y técnicos que requieren atención.


The field of radiology has seen notable advances in recent decades, with developments ranging from image quality improvement and digitization to computer-aided detection. Particularly, the emergence of Artificial Intelligence techniques based on Deep Learning and Computer Vision have promoted innovative solutions in diagnosis and radiological analysis. This article explores the relevance of open source developments and models in the progress of these techniques, highlighting the impact that collaboration and open access have had on the scientific advancement in this field. This research has a qualitative approach, with a descriptive, retrospective, longitudinal scope. A documentary analysis of the evolution and impact of open source in Radiology was carried out, highlighting multidisciplinary collaboration. Use cases, advantages, challenges and ethical considerations were also examined in relation to the implementation of AI-based solutions in Radiology. The Open Source approach has been shown to be a positive influence in Radiology, with the potential to influence medical care, offering more precise and accessible solutions. However, there are ethical and technical challenges that require attention.

RESUMEN Las técnicas de neuroimagenología otorgan información relevante del estado funcional y anatómico del cerebro humano. Esta información es particularmente importante cuando existe una lesión cerebral causada por alguna patología, tal como la enfermedad vascular cerebral (EVC). En pacientes afectados por esta enfermedad, se ha determinado que la neuroplasticidad es el mecanismo principal de recuperación de la función motora perdida. Debido a la alta prevalencia de la EVC a nivel mundial y especialmente en países en vías de desarrollo, es necesario continuar investigando los mecanismos de recuperación involucrados en esta patología. La resonancia magnética funcional (RMF) y la imagenología por tensor de difusión (ITD) son dos de las técnicas de neuroimagenología más utilizadas con este fin. La RMF permite analizar la actividad neuronal generada al ejecutar tareas de movimiento, mientras que la ITD proporciona información estructural de la anatomía cerebral. En esta revisión narrativa, se presentan diversos estudios que han utilizado estas técnicas de neuroimagenología en la cuantificación de los cambios de neuroplasticidad en pacientes con EVC tras participar en algún programa de neurorrehabilitación. Comprender mejor estos cambios de neuroplasticidad permitiría diseñar esquemas de rehabilitación que proporcionen un mayor beneficio a los pacientes con EVC.


ABSTRACT Neuroimaging techniques provide relevant information of the functional and anatomical status of the human brain. This information is of particular importance when a pathology, like stroke, produces a brain injury. In stroke patients, it has been determined that neuroplasticity is the primary recovery mechanism of the lost motor function. Due to worldwide high prevalence, especially in developing countries, it is necessary to continue the research of the recovery mechanisms involved in this pathology. To this end, functional magnetic resonance imaging (fMRI) and diffusion tensor imaging (DTI) are two of the most used neuroimaging techniques. In stroke patients, fMRI allows the analysis of the neural activity produced by the execution of motor tasks, whereas DTI provides structural information of the brain anatomy. In this narrative review, multiple studies that employ these neuroimaging techniques for quantification of neuroplasticity changes in stroke patients after undergoing a neurorehabilitation program are presented. Better understanding of these neuroplasticity changes would allow researchers to design and provide more beneficial rehabilitation schemes to stroke patients.

Lung function testing is an essential modality of investigation in children as it provides objective evidence of lung disease/ health. With advances in technology, various tests are available that can aid in the diagnosis of lung disease, assess the progression and response to therapy and document the lung development and evolving lung diseases in infants. This narrative review discusses lung function tests in infants and children. Currently, lung function tests can be performed in every age group, from neonates to the elderly. Spirometry and peak expiratory flow rate (PEFR) are the most employed tests in children more than six years of age. Spirometry helps diagnose and monitoring of both obstructive and restrictive diseases. There is a need for expertise to perform and interpret spirometry correctly. The forced oscillation technique (FOT) or impulse oscillometry (IOS) is done with tidal volume breathing and is feasible even in preschool children. Their utility is mainly restricted to asthma in children at present. Lung function tests can be performed in neonates, infants and children using infant pulmonary function test (PFT) equipment, although their availability is limited. Diffusion capacity for carbon monoxide (DLCO) is a valuable tool in restrictive lung diseases. Lung volumes can be assessed by body plethysmography and multiple washout technique. The latter can also assess lung clearance index. It is essential to perform and interpret the lung function test results correctly and correlate them with the clinical condition for optimum treatment and outcome.

Introducción: El trastorno afectivo bipolar (TAB) se ha asociado con una disminución de la integridad de la sustancia blanca. Los estudios con imágenes con tensor de difusión (DTI) han permitido elucidar con una mayor calidad estos cambios. Debido a la gran heredabilidad del TAB, se han realizado estudios en familiares de pacientes con TAB acerca de la integridad de la sustancia blanca, y se ha encontrado que la conectividad estructural también puede estar afectada. Dicha alteración se ha propuesto como un potencial biomarcador de vulnerabilidad a este trastorno. Sin embargo, los estudios en niños y adolescentes son pocos. Objetivo: Revisar la literatura sobre los cambios en la integridad de la sustancia blanca determinados mediante DTI en niños y adolescentes con alto riesgo. Resultados: Se describe la conectividad estructural cerebral en la población pediátrica en estudios que utilizaron DTI. Se describen los cambios en el proceso de mielinización desde su evolución dentro del neurodesarrollo normal hasta los hallazgos en la anisotropía fraccional (AF) en pacientes con TAB y los familiares en alto riesgo. Conclusiones: Los estudios demuestran que tanto pacientes con TAB como sus familiares en riesgo presentan disminución de la AF en regiones cerebrales específicas. Los estudios en niños y adolescentes con riesgo familiar de TAB señalan una AF reducida en tractos axonales implicados en funciones emocionales y cognitivas. La disminución de la AF puede considerarse como un biomarcador de vulnerabilidad al TAB.


Introduction: Bipolar disorder (BD) has been associated with a decrease in white matter integrity. Diffusion tensor imaging (DTI) studies have enabled these changes to be elucidated with higher quality. Due to BD's high heritability, some studies have been conducted in relatives of BD patients looking at white matter integrity, and have found that structural connectivity may also be affected. This alteration has been proposed as a potential BD biomarker of vulnerability. However, there are few studies in children and adolescents. Objective: To conduct a review of the literature on changes in white matter integrity determined by DTI in high-risk children and adolescents. Results: Brain structural connectivity in the paediatric population is described in studies using DTI. Changes in the myelination process from its evolution within normal neurodevelopment to the findings in fractional anisotropy (FA) in BD patients and their high-risk relatives are also described. Conclusions: Studies show that both BD patients and their at-risk relatives present a decrease in FA in specific brain regions. Studies in children and adolescents with a high risk of BD, indicate a reduced FA in axonal tracts involved in emotional and cognitive functions. Decreased FA can be considered as a vulnerability biomarker for BD.

Background: Stroke is a prevalent and potentially fatal medical condition that affects individuals worldwide. Ischemic strokes, caused by arterial blockages, are the most common type, accounting for about 80% of all cases. Hemorrhagic strokes, on the other hand, are less frequent but can have more severe consequences. Accurate and timely diagnosis of stroke is critical for effective treatment and optimal patient outcomes. In this context, diffusion-weighted imaging (DWI) has emerged as a valuable tool for identifying and monitoring ischemic stroke. This article provides an overview of the role of DWI in stroke assessment and management, with a focus on early detection and intervention. The aim of this study is to investigate the reliability of diffusion-weighted MRI (DWI) as an imaging modality in the evaluation of acute ischemic stroke. Material & Methods: This prospective cross-sectional study was conducted at the Department of Radiology and Imaging at the Combined Military Hospital (CMH) in Dhaka, Bangladesh, between June 2020 and June 2021. The study aimed to examine 120 patients clinically diagnosed with acute ischemic stroke. Prior to the study, verbal consent was obtained from all patients. The Study subjects were identified in the emergency and casualty department and had undergone an MRI of the brain in the Department of Radiology and Imaging at CMH, Dhaka. . Data were collected on pre-designed forms, and the relevant information was compiled on a master chart for statistical analysis using SPSS software. Descriptive analysis and frequency of results were presented in the form of tables, pie charts, and bar graphs. The ethical clearance of this study was obtained from the Institutional Ethics Committee of CMH, Dhaka, Bangladesh. Results: The study included 64 males and 56 females with a mean age of 65.2 ± 7.83 years. The majority of patients had an acute ischemic lesion (88.30%) in the MCA (50.0%) with small-sized lesions (41.7%) and low ADC values (90.0%). The most common clinical presentation was hemiplegia (100%), and diffusion-weighted MRI had a high diagnostic accuracy in detecting acute ischaemic lesions (96.3%). Overall, these findings highlight the importance of MRI in the evaluation of stroke patients and can guide clinical decision-making. Conclusion: The present study concluded that DWI in conjunction with ADC map MR imaging is a gold standard diagnostic modality in the evaluation and management of acute ischaemic stroke.

Background: Brewer ́s spent grain (BSG) is a biomass by-product generated in large volumes during industrial beer production. BSG has become a growing environmental problem, as most breweries discard it inappropriately, negatively impacting the environment. Alternatives for the exploitation of this by-product have consisted of elaborating food supplements for farm animals, obtaining biofuels, developing adsorbents, and obtaining substances for the food industry. However, the high moisture content in BSG (approximately 70%), poses a significant challenge in exploring various reuse alternatives. Therefore, the implementation of a pre-drying process becomes essential. Objective: This study aimed to analyze the BSG drying kinetics at different temperatures and the effect of the drying temperature on the physical properties and the content of bioactive compounds. Methods: BSG samples were dried at different temperatures (50, 60, 70, 80, 90, and 105°C) and analyzed for their moisture ratio, water activity, total polyphenol content (TPC), and DPPH (1,1-diphenyl-2-picrylhydrazil) radical scavenging activity. Also, four kinetics models were fitted to the drying data. Results:It was determined that the effective diffusivity was between 5.23x10


Antecedentes: El Bagazo residual de malta (BSG por sus siglas en inglés) es un subproducto biomásico generado en grandes volúmenes durante la producción industrial de cerveza. El BSG se ha convertido en un creciente problema para el medio ambiente, debido a que la mayoría de las cervecerías descartan inapropiadamente este residuo generando un impacto negativo al ambiente. Las alternativas para el aprovechamiento de este subproducto han consistido especialmente en la elaboración de suplementos alimenticios para animales de granja, obtención de biocombustibles, desarrollo de adsorbentes y obtención de productos para la industria alimentaria. Sin embargo, el alto contenido de humedad (~70%) del BSG representa un reto para el desarrollo de diferentes alternativas de reutilización, por lo que se hace necesario un proceso de secado previo. Objetivos: En este estudio se analizó la cinética de secado del BSG a diferentes temperaturas y el efecto de la temperatura de secado sobre sus propiedades físicas y contenido de compuestos bioactivos. Métodos: Las muestras de BSG fueron secadas a diferentes temperaturas (50, 60, 70, 80, 90 y 105°C) y analizadas en términos de razón de humedad, actividad acuosa, contenido de polifenoles totales (TPC) y actividad secuestradora del radical DPPH. Además, se ajustaron 4 modelos cinéticos a los datos de secado. Resultados: Se determinó que la difusividad efectiva del BSG varió entre 5.23x10

ABSTRACT

Background: Wound infection comprises numerous different organisms that have the ability to surface colonization of wounds. Multidrug-resistant Pseudomonas aeruginosa is one of the pathogenic bacteria associated with wound infections. Aim: This study isolated and identified multidrug-resistant Pseudomonas aeruginosa from infected wounds and determine the antibacterial activity of Lawsonia inermis leaf extracts against it. Design: This is a Clinical and laboratory-based study involving patients with defined cases of wound infections. Place and Duration of Study: This study was conducted in the Microbiology (Bacteriology) laboratory of Specialist Hospital, Bauchi, Nigeria, from February to November 2021. Methods: Twenty-eight (28) Pseudomonas aeruginosa isolates were recovered from 179 wound swabs using standard laboratory procedures and were screened for multidrug-resistant patterns according to the Kirby-Bauer disc diffusion method. Antibacterial efficacy of the aqueous, ethanolic, and methanolic leaf extracts of Lawsonia inermis was tested against the multidrug-resistant isolates using agar well diffusion techniques. The zone of inhibition was measured and the differences between means were statistically analyzed (p<0.05). Results: A total of twenty-eight (28) multidrug-resistant Pseudomonas aeruginosa were confirmed, showing resistance to Amoxicillin (64.3%), Ceftazidime (85.71%), and Cefotaxime (78.57%) but sensitivity to Imipenem (95.5%). The phytochemical screening revealed the presence of flavonoids, glycosides, saponins, steroids, and tannins among others. MDR P. aeruginosa was inhibited at varied concentrations of the extracts with the diameter mean zone of inhibition increasing as the concentration increased. The Methanol extracts showed the highest antibacterial activity against MDR P. aeruginosa with a mean zone of inhibition of 9.500±0.288mm at 400mg/ml. Conclusion: These results indicated that Lawsonia inermis leaf extracts possess antibacterial activities on Multidrug-resistant Pseudomonas aeruginosa which could be a good source for the production of plant-based antibacterial drugs., although somewhat less than the synthetic standard drugs (Imipenem) having a mean of 13.83±0.288mm.

SUMMARY OBJECTIVE: The aim of this study was to evaluate the diagnostic performance of diffusion-weighted imaging compared to non-contrast magnetic resonance imaging in the differential diagnosis of acute appendicitis in pregnant patients. METHODS: A total of 72 pregnant patients with the suspicion of acute appendicitis who underwent magnetic resonance imaging combined with diffusion-weighted imaging examinations were enrolled in this retrospective study. Magnetic resonance imaging images (non-contrast and diffusion-weighted imaging sequences) were evaluated. Moreover, apparent diffusion coefficient ratios were estimated. The diagnostic performances of magnetic resonance imaging and diffusion-weighted imaging findings were statistically analyzed on the basis of surgical and follow-up results. RESULTS: Of 72 pregnant patients, 10 (14%) had acute appendicitis on magnetic resonance imaging and diffusion-weighted imaging. Among 10 patients with acute appendicitis, three (3/10) had perforation. diffusion-weighted imaging findings had higher sensitivity (90 versus 60%), negative predictive value (98.41 versus 93.94%), and accuracy (98.61 versus 94.44%) ratios compared to non-contrast magnetic resonance imaging in the diagnosis of acute appendicitis. There was one false-negative result on diffusion-weighted imaging. Diffusion restriction facilitated the detection of appendicitis. The apparent diffusion coefficient ratios were lower in acute appendicitis than in the normal appendix (0.70±0.19 versus 0.96±0.16) (p<0.05). CONCLUSION: With a shorter scan time and higher diagnostic accuracy, diffusion-weighted imaging can be useful for the early diagnosis of acute appendicitis and for planning appropriate management.

Resumen El ataque cerebrovascular (ACV) es la primera causa de discapacidad y la tercera causa de muerte en la Argentina y en el mundo. La etiología del stroke es isquémica en su mayoría (más del 80% de los casos), siendo las causas más frecuentes las lesiones isquémicas de pequeños vasos de tipo lacunar y las tromboembólicas debido a ateroesclerosis de grandes arterias o cardioembolia. Las secuencias utilizadas en nuestro protocolo de estudio incluyen DWI, FLAIR, angio-RM 3D-TOF, SWI y FSE-T1 (tiempo aproximado de exploración de 13 minutos). Pueden observarse diferentes hallazgos en cada una de estas secuencias que son orientativos para determinar la etiología subyacente y estimar el tiempo de evolución de la lesión. En la actualidad, la trombectomía mecánica se ha convertido en el tratamiento de elección en pacientes seleccionados con accidente cerebral isquémico agudo. La transformación hemorrágica es la complicación más temida luego de la terapia de reperfusión intraarterial. La técnica de difusión de la resonancia magnética (RM) posee una mayor sensibilidad para detectar lesiones isquémicas, permite sospechar la etiología según el patrón lesional y aporta información valiosa para la selección de los pacientes candidatos a la reperfusión arterial.


Abstract Stroke or cerebrovascular attack (CVA) is the first cause of disability and the third cause of death in Argentina and in the world. The etiology of strokes is mostly ischemic (more than 80% of cases), with the most frequent causes being lacunar-type ischemic lesions of small vessels and thromboembolic lesions due to atherosclerosis of large arteries or cardioembolism. The sequences used in our study protocol include DWI, FLAIR, angio-RM 3D-TOF, SWI, and FSE-T1 (approximate scan time of 13 minutes). Different findings can be observed in each of these sequences that are indicative to determine the underlying etiology and estimate the time of evolution of the lesion. Currently, mechanical thrombectomy has become the treatment of choice in selected patients with acute ischemic stroke. Hemorrhagic transformation is the most feared complication after intra-arterial reperfusion therapy. The diffusion magnetic resonance technique is more sensitive for detecting ischemic lesions, allows one to suspect the etiology based on the lesion pattern, and provides valuable information for the selection of patients who are candidates for arterial reperfusion.

Resumen Los cambios políticos sucedidos en Bolivia en los últimos 50 años llevaron a modificaciones en la participación de grupos subalternizados y en la apreciación ética de la vida. Este estudio pretende establecer las conductas bioéticas de la cosmovisión andina en aculturados, endoculturados y transculturizados en La Paz, Bolivia. Se trata de una investigación mixta, exploratoria, etnográfica, transversal, comparativa sincrónica, diacrónica y sistemática, en la cual se aplicó un cuestionario validado por expertos (alfa de Cronbach de 0,932) a 399 personas (5,3% aculturadas; 73,9%, endoculturadas y 20,8%, transculturalizadas). Los resultados contrastados por hermenéutica obtuvieron cinco categorías, de las cuales la categoría relacionalidad era independiente a la culturación (p=0,262); mientras que las categorías conciencia ecológica, complementariedad, reciprocidad, concepto de salud, modelo social y comunitario dependían entre sí (p=0,000). Se constató que los sujetos aculturados del área rural presentaban mayor conocimiento de la ética ancestral. Se recomienda la promoción de políticas educativas relacionadas a ética.


Abstract The political changes that have taken place in Bolivia in the last fifty years have led to modifications in the participation of subalternized groups and changes in the ethical appreciation of life. This study aims to establish the bioethical behaviors of the Andean cosmovision in acculturated, endoculturated and transculturated people in La Paz, Bolivia. It is a mixed, exploratory, ethnographic, cross-sectional, comparative, synchronic, diachronic and systematic research, in which a questionnaire validated by experts (Cronbach's alpha 0.932) was applied to 399 people (5.3% acculturated; 73.9%, endoculturated and 20.8%, transculturated). The results contrasted by hermeneutics obtained five categories, of which the category relationality was independent of acculturation (p=0.262); while the categories ecological awareness, complementarity, reciprocity, concept of health, social model and community depended on each other (p=0.000). It was found that the acculturated subjects from the rural area presented greater knowledge of ancestral ethics. The promotion of educational policies related to ethics is recommended.


Resumo As mudanças políticas que ocorreram na Bolívia nos últimos 50 anos levaram a mudanças na participação de grupos subalternizados e na apreciação ética da vida. Este estudo tem como objetivo estabelecer os comportamentos bioéticos da cosmovisão andina em pessoas aculturadas, endoculturadas e transculturadas em La Paz, Bolívia. Resumo Este artigo tem como objeto de análise a implantação da Lei Complementar nº 123, de 2006 (LC nº 123/2006), que instituiu o Estatuto Nacional da Microempresa e da Empresa de Pequeno Porte (MPE), por municípios paulistas. Utilizou-se o referencial teórico de difusão de políticas públicas tendo como objetivo estudar seu processo de adoção, observando os diferentes contextos e momentos de coerção previstos na legislação federal. Com base nas discussões teóricas relacionadas com o mecanismo de difusão chamado de coerção, respondeu-se a seguinte pergunta: qual é a influência do mecanismo de coerção vertical na difusão da Lei Geral da MPE nos municípios paulistas? A técnica "análise de sobrevivência" foi aplicada para identificar fatores explicativos da difusão da política, considerando variáveis de desenho institucional, políticas e partidárias, efeito vizinhança, necessidade da política, organizações/atores sociais e fatores estruturais. Os resultados demonstram que, conforme varia o desenho institucional de coerção, mudam os fatores explicativos da difusão dessa política na ponta, de modo que os aspectos locais, incluindo atores sociais, fatores regionais e questões estruturais, importam para a difusão, variando conforme o contexto coercitivo.


Resumen Este artículo analiza la implementación de la Ley General de las Micro y Pequeñas Empresas por parte de los municipios de São Paulo. Se utilizó la teoría de la difusión de políticas, con el objetivo de estudiar el proceso de adopción de esta política pública, observando los diferentes contextos y momentos de coerción previstos en la legislación federal. A partir de las discusiones teóricas relacionadas con el mecanismo de difusión denominado coerción, se respondió a la siguiente pregunta: ¿cuál es la influencia del mecanismo de coacción vertical en la difusión de la mencionada ley en los municipios de São Paulo? Se aplicó la técnica de análisis de supervivencia para identificar factores explicativos de la difusión de políticas, considerando variables institucionales, políticas y partidarias, efecto barrio, necesidad de políticas, organizaciones/actores sociales y factores estructurales. Los resultados muestran que a medida que varía el diseño institucional de coerción, los factores explicativos de la difusión de esta política al final cambian, de modo que los aspectos locales, incluidos los actores sociales, los factores regionales y los problemas estructurales importan para la difusión, variando de acuerdo con el contexto coercitivo.


Abstract This article analyzes the implementation of the General Law for Micro and Small Business by municipalities in São Paulo. The theory of policy diffusion was used to study the process of adopting this public policy in the different contexts of coercion. The article presents theoretical discussions on the diffusion mechanism "coercion" and answers the question: what is the influence of the vertical coercion mechanism in the diffusion of the General Law in the municipalities of São Paulo? The survival analysis technique was applied to identify explanatory factors of policy diffusion, considering institutional, political, and party design variables, neighborhood effect, internal needs, organizations/social actors, and structural factors. The results show that as the institutional rules of coercion vary, the explanatory factors of the diffusion change so that local aspects, including social actors, regional factors, and structural issues, matter for the diffusion, varying according to the coercive context.

Resumo O presente estudo busca contribuir para a melhor compreensão de processos de difusão de política com base em arenas transnacionais - mais especificamente, do processo de difusão que influenciou a regulação das Boas Práticas de Fabricação (BPF) de medicamentos no Brasil num contexto envolto por atores internacionais. Por meio de pesquisa qualitativa, analisamos o processo de adesão da Agência Nacional de Vigilância Sanitária (Anvisa) ao arranjo de cooperação Pharmaceutical Inspection Co-operation Scheme (PIC/S), iniciado em 2010 e alcançado em 2021. Foi identificado um processo influenciado por 2 constelações de difusão, motivado por interesses da agência nacional em manter sua relevância e por atores que integram o Sistema Nacional de Vigilância Sanitária, no qual o modelo de equivalência e convergência regulatória do PIC/S se mostrou fundamental para a adaptação da referência internacional em nível nacional, mantendo o sistema em funcionamento. Tal processo de difusão de política ficou mais relevante nos últimos anos por ampliar a convergência regulatória e, potencialmente, tornar mais eficiente a avaliação das BPF de medicamentos pelas diversas autoridades sanitárias.


Resumen El presente estudio pretende contribuir a una mejor comprensión de los procesos de difusión de políticas, más concretamente, del proceso de difusión que influyó en la regulación de las Buenas Prácticas de Fabricación (BPF) de medicamentos en Brasil, a partir de arenas transnacionales. A través de una investigación cualitativa, analizamos el proceso de adhesión de la Agencia Nacional de Vigilancia Sanitaria (Anvisa) al Esquema de Cooperación de Inspección Farmacéutica (PIC/S), iniciado en 2010 y alcanzado en 2021. Se identificó un proceso influenciado por dos constelaciones de difusión, motivado por el interés de la agencia nacional en mantener su relevancia y por los actores que componen el Sistema Nacional de Vigilancia Sanitaria, en el que el modelo de equivalencia y convergencia normativa del PIC/S resultó fundamental para la adaptación de la referencia internacional al ámbito nacional, manteniendo el sistema nacional en funcionamiento. Este proceso de difusión de políticas adquirió aún más relevancia en años recientes al ampliar la convergencia normativa y hacer potencialmente más eficiente la evaluación de las BPF de los medicamentos por parte de las distintas autoridades sanitarias.


Abstract The present study seeks to contribute to a better understanding of policy diffusion processes, more specifically, of the diffusion process from a transnational arena that influenced the regulation of Good Manufacturing Practices (GMP) for medicinal products in Brazil in a context surrounded by international authorities. By conducting qualitative research, we analyzed the process of adhesion of the Brazilian Health Regulatory Agency (Anvisa) to the Pharmaceutical Inspection Co-operation Scheme (PIC/S), initiated in 2010 and achieved in 2021. A process influenced by two constellations of diffusion was identified, motivated by the national agency's interests in maintaining its relevance and by actors that make up the National Sanitary Surveillance System, in which the PIC/S model of regulatory equivalence and convergence proved to be fundamental for the adaptation of the international reference to the national level, keeping the national system functioning. Such a policy diffusion process became even more relevant in the past years due to the expansion of regulatory convergence and potentially making the various health authorities' GMP assessment of medicinal products more efficient.

ABSTRACT

SUMMARY OBJECTIVE: The aim of this study was to investigate the correlation of fibrosis stages in cases of chronic hepatitis by comparing shear wave elastography and diffusion-weighted magnetic resonance imaging. METHODS: A total of 46 chronic hepatitis patients with an age range of 20-50 years were classified into three groups based on their fibrosis stages. Comparison group 1: the presence of fibrosis (S0 and S1≤); comparison group 2: the presence of significant fibrosis (≤S2 and S3≤); and comparison group 3: the presence of cirrhosis (≤S4 and S6). Shear wave velocities were measured by acoustic radiation force impulse elastography. Diffusion-weighted magnetic resonance imaging was performed on a 3.0 Tesla MRI device. RESULTS: In comparison group 1 (S0 and S1≤), the area under the curve, sensitivity, and specificity of acoustic radiation force impulse values were 0.784, 87, and 60%, respectively, while these values were 0.718, 80, and 66%, respectively, for apparent diffusion coefficient . In comparison group 2 (≤S2 and S3≤), the area under the curve, sensitivity, and specificity of acoustic radiation force impulse values were 0.917, 80, and 86%, respectively, and the apparent diffusion coefficient values were 0.778, 90, and 66%, respectively. In comparison group 3, the area under the curve, sensitivity, and specificity of acoustic radiation force impulse values were 0.977, 100, and 95%, respectively. There was no statistically significant difference between the apparent diffusion coefficient values of the cases in the three groups (p=0.132). CONCLUSION: Noninvasive methods are gaining importance day by day for staging hepatic fibrosis. Acoustic radiation force impulse elastography was evaluated as a more reliable examination than diffusion-weighted magnetic resonance imaging in revealing the presence of fibrosis, determining significant fibrosis, and diagnosing cirrhosis.

Objetivo: investigar o estado da arte das publicações científicas ampliadas voltadas para a dinamização da ciência. Método: pesquisa qualitativa de cunho integrativo nas seguintes bases de dados Web of Science, Library, Information Science and Technology Abstracts e Scopus via Portal Periódicos Capes, Dimensions, PLOS ONE, Base de Dados Referencial de Artigos de Periódicos em Ciência da Informação e Scientific Eletronic Library Online no período de junho de 2022. Resultados: onze artigos divididos em duas categorias: caracterização das publicações ampliadas e infraestrutura de suporte para as publicações ampliadas. Conclusão: apesar de multivariadas, as publicações ampliadas possuem características peculiares que as tornam objetos digitais compostos e dinâmicos e com melhor experiência para o usuário. Os artigos apontam que não há infraestrutura de suporte nas plataformas de periódicos e quando encontrado, se apresenta simples e com poucos recursos.


Objectives: to investigate the state of the art of expanded scientific publications aimed at boosting science. Method: qualitative research of an integrative nature in the following databases: Web of Science, Library, Information Science and Technology Abstracts and Scopus via Portal Periodicals Capes, Dimensions, PLOS ONE, Reference Database of Journal Articles in Science of Information and Scientific Electronic Library Online in the period of June 2022. Results: eleven articles divided into two categories: characterization of expanded publications and support infrastructure for expanded publications. Conclusion: Despite being multivariate, expanded publications have peculiar characteristics that make them composite and dynamic digital objects with a better user experience. The articles point out that there is no support infrastructure on journal platforms and when found, it is simple and with few resources.

Conclusión: a pesar de ser multivariadas, las publicaciones expandidas tienen características peculiares que las convierten em objetos digitales compuestos y dinâmicos com uma mejor ecperiencia de usuário. Los artículos señalan que no existe uma infraestructura de apovo em las plataformas de revistas y cuando se encuentra es sencilla y com pocos recursos.
